For men and women in their 40s and 50s who train — and still can't lose the belly

You're training. You're eating right. The belly fat still isn't moving.

That's not a willpower problem — after 40, it's usually a metabolic one. Optima is a doctor-led program built around retatrutide — an investigational medication studied for exactly this problem — with a plan designed to protect the muscle you've worked for.

"I work out six days a week. I don't think it's a lack of effort."What we hear on almost every consultation

In your free evaluation, Dr. Medina reviews your history, your goals and your labs if you have them — and tells you honestly whether this program fits you. If it doesn't, he'll say so.

Does this sound familiar?

You've done everything "right."
And it's still not working.

"I'm eating better than ever and the scale won't budge."

"I train hard — but the last 20–30 pounds don't respond to effort anymore."

"I've lost weight before. I could never keep it off."

The answer isn't trying harder. It's a treatment that works with your metabolism — supervised by a doctor who checks whether it's right for you first.

Who's actually treating you

A real doctor. His cell number. The whole program.

This isn't a telehealth mill where a stranger rubber-stamps a prescription. Dr. Medina runs every evaluation himself — a 30-minute conversation about your history, your lifestyle and your labs (he runs long when your case needs it) — and only clears you for treatment he'd give his own family.

Once you're in a program, you have his direct line. Side effect at 8pm? Question about a dose? You text the doctor, not a ticket queue.

Every prescription is issued under Mexican medical license after your individual evaluation — on video from home, or in person at the clinic. Labs, imaging and visits on the 25th floor are there whenever you want them.

Why effort stops working

After 40, your hormones change the rules.

Appetite signals get louder. Insulin resistance creeps up. Recovery slows. The same food and the same training that kept you lean at 30 quietly stop working — especially around the belly, where the most dangerous fat (visceral fat) collects.

Retatrutide is an investigational medication being studied for exactly this problem. Unlike single-pathway GLP-1 drugs, it acts on three receptors at once — GIP, GLP-1 and glucagon — which is why researchers call it a "triple agonist."

In Eli Lilly's published phase 2 trial (New England Journal of Medicine, 2023), participants on the highest dose saw an average weight reduction of 17.5% at 24 weeks and 24.2% at 48 weeks. Retatrutide remains investigational and is not approved by the FDA; results vary and no outcome is guaranteed.

GLP-1 GIP GCG All three at once

What the three pathways do

1

GLP-1 — appetite

Studied for quieting food noise and cravings — so a normal portion actually satisfies.

2

GIP — insulin response

Supports how your body handles blood sugar, the engine of stubborn-fat storage.

3

Glucagon — energy burn

The third pathway, being studied for increased energy expenditure — the piece single-pathway drugs don't touch.

The fear everyone brings us

"I don't want to look like a stick."

You've seen it: friends on weight-loss shots who lost the weight and their muscle, their strength, their face. It's the #1 concern patients raise with Dr. Medina — and it's the reason this program is built the way it is.

"I want to lose weight, get lean — but I don't want to lose any of my muscle mass."Patient, 46 — initial consultation

muscle stays with you slow dosetitration proteintargets resistance training

How the program defends your muscle

  • Start low, titrate slowly — doses rise only when your body tolerates them, so you can keep training through the program.
  • Protein + resistance plan — the doctor sets protein targets and resistance work with you, because medication alone doesn't protect muscle.
  • Overshoot + taper — we plan past your goal weight and wean off gradually, so the bounce-back doesn't erase your result.
  • Follow-ups that adjust — weight, appetite and side effects reviewed at every check-in; your dose changes when the data says so.
How it works

From "nothing works" to a plan — in three steps.

1 · See if you're a candidate

Take the 2-minute quiz. It mirrors the doctor's own intake questions — so your evaluation starts warm, not from zero.

2 · Free video evaluation

30 minutes with Dr. Medina from your home. History, goals, labs if you have them. If you're not a fit, he tells you straight.

3 · Your plan + first shot together

If you're cleared, you get a written treatment plan and a complete kit — and the doctor does your first injection with you on video.

Good to know before we begin

Frequently asked questions

Is retatrutide FDA-approved?

No. Retatrutide is an investigational medication currently in phase 3 clinical trials and is not approved by the FDA or COFEPRIS. Your evaluation — and any prescription — happens under Dr. Medina’s Mexican medical license at the Tijuana clinic. During your consultation, Dr. Medina explains exactly what that means for you — including the published trial data and the risks — so you can decide with full information.

Will I lose muscle, or end up with "Ozempic face"?

This is the #1 concern we hear, and it's why the program pairs the medication with protein targets, resistance training and slow titration. Muscle protection comes from how the program is run — dose, food and training — not from the medication alone. Patients who keep training through the program are exactly who this is designed for.

I had a terrible time on Ozempic / Wegovy. Why would this be different?

Many of our patients came to us after a rough experience on single-pathway GLP-1 drugs. Dr. Medina starts every patient at a deliberately low dose, moves up only as your body tolerates it, and is a text away if side effects show up. GI side effects (nausea, constipation or loose stool) are the most common and usually appear during dose increases — which is when your dose gets adjusted, not pushed.

Do I have to stay on it forever?

No. Programs run in three phases — a loading month, a weight-loss phase, then a maintenance taper — and patients are weaned off gradually. We plan slightly past your goal weight because some bounce-back is normal. The longest we keep anyone on treatment is 18 months; most patients run 3–6 months, with the core phase around 3.

I've never injected myself. Is that a problem?

Almost nobody has. Your kit comes pre-prepared, and Dr. Medina does your first shot with you on a video call. It's a tiny insulin-style needle into the fatty tissue around your stomach, and the doctor does the first one with you on video.

Do I have to come to Tijuana?

Not to start. Your evaluation happens on a video call, and most patients run their whole program virtually. The Tijuana clinic (25th floor, Zona Urbana Río) is there when you want it — labs, imaging, in-person visits — and many SoCal patients cross for a same-day lab visit.

How does my kit actually reach me in the U.S.?

Your kit is prepared at the Tijuana clinic, pre-measured and packed with everything you need. Most Southern California patients pick it up or meet the clinic team in person. Outside SoCal, the clinic confirms your exact delivery option with you — in writing, before you pay anything.

Why a Tijuana clinic instead of a U.S. telehealth site?

Because here a real, named physician is accountable to you. U.S. sites selling "research peptides" ship vials with no doctor behind them. At Optima, a licensed physician (Cédula 8428907) evaluates you first, your plan is in your chart, the clinic is a physical place you can walk into, and the doctor stays a text away for your whole program.

How much does it cost?

The evaluation with the doctor is free. If you're cleared and want to move forward, programs are priced by treatment plan — you'll see exact numbers in writing before you pay anything, and there's no obligation after the consultation.

Is this safe? Who's accountable?

Every patient is evaluated by Dr. Medina (Professional License 8428907) before anything is prescribed, and no treatment happens without a medical history review. You keep direct access to the doctor for the length of your program, and follow-ups track your weight, response and side effects at every step.
